The IncidentMgr software allows organizations to report near miss, high potential, injuries, unsafe conditions and more electronically very quickly and efficiently. This data is then stored into an electronic database and information is shared with key stakeholders. Once a Preliminary Incident Notification (PIN) is created, an investigation leader is assigned along with a team to appropriately conduct the proper Root Cause Analysis (RCA).


The software includes a built-in Root Cause Analysis tool that allows the RCA leader to map the causes and effects of the problem accurately based on precise evidence leading to root causes and contributing root causes where appropriate recommendations are then assigned to prevent re-occurrence.


A comprehensive report is then generated with a click of a button and sent to key managers for approval electronically. Once approved, the recommendations are then linked to the TASK Mgr portal where each employee can clearly see the recommendation as a task, the incident associated with the task, the team that worked on the report and the approvers. This information is critical should the task owner require follow-up with the key individuals knowledgeable of the incident. A priority and due date allows the task owner to efficiently work to complete the job and update the task electronically until he/she is able to close the recommendation on time. Along the way, every time the task owner updates the task progress, the incident leader is then able to approve, reject or request more details on the update to ensure the recommendation is implemented appropriately as to prevent re-occurrence of the incident. The approval process is electronic with a time stamp and clear work flow. Feedback from the task owner to task stakeholder is in real time and can be completed from any computer, laptop, or hand held device. You no longer need to wait for your manager to get free to discuss updates; this can be done remotely from virtually any location.


This integrated software is key to ensuring reporting of all unsafe conditions, near misses, injuries, design issues…etc, but more importantly these reported items get the proper investigation attention and recommendation to correct them are appropriately assigned, worked on, approved, and closed on time to help improve the overall PSM implementation score.
